Transition Activation Worker
The Transition Activation Worker Program provides a source of funding to support individuals up to 18 years of age with a serious Mental Health illness as they transition back into their community.
The Transition Activation Worker Program supports children and youth by providing:
- Short term funding for a trained worker to support the individual in the community upon discharge from hospital/residential treatment.
- Flexibility to allow scheduling of interactions that meet the child/youth’s needs.
- Monitoring to ensure that the treatment setting provides the clinical supervision.
- A close working relationship with Mental Health service providers to ensure opportunities for success in reaching the predetermined goals.
How can I make a referral?
- Hospital or residential treatment facility must complete the application for funding on behalf of the client on an ongoing basis.
- Applications are reviewed as received.
- Decisions are made based on criteria and availability of funding and are communicated to the service provider and family.
