Co-ordination Services for Children and Youth (CSCY)
Co-ordination Services for Children and Youth (CSCY) provides support when the family has experienced a breakdown in services due to the complicated needs of the child/youth. The program is available to individuals from 0 to their 18th birthday and their families whose needs cannot be met by the existing service system.
Co-ordination Services for Children and Youth provides:
- Unique service plans which utilize the strengths of community services to address short and long term development.
- Access point for Durham clients to Ministry funded Central East Region programs and services.
- Hard to Serve Funding that supports short term goals identified on a service plan or to respond to immediate crisis.
- Consultation to service providers to support them in meeting the needs of complex clients.
How do I make a referral?
- Parent or service provider calls 905-427-7207 and leaves a message as directed.
- A Service Coordinator will make first contact with the parent/guardian within seven (7) business days.
- A meeting will be booked to determine service needs and next steps.
