Consultation Program
The Consultation Program services are available to support children 2 to 12 years of age with an identified need. Services are provided on a flexible schedule to home and community programs including child care centres, camps, and school boards.
The Consultation Program supports the child’s inclusion into the community by providing:
- A collaborative approach to service delivery with other service providers, sharing expertise and resources to ensure a cohesive plan to meet the needs of the child.
- Ongoing contact including site visits to discuss services and information regarding the plans for the child as initiated or agreed upon by the family.
- Monitoring of developmental progress and provision of programming to enhance skills as identified by the family.
- Case management to assist families in accessing other programs/services.
How can I make a referral?
- Referrals will be received by phone from parent/guardian or service provider with parental permission.
- To leave a Referral voicemail, contact (905) 427-8862 ext. 541 and follow the directions.
- The Intake Coordinator will contact the parent/guardian within two business days to book a meeting or teleconference to talk in detail about the family’s concerns and the services required.
